Proof of Yuma County Residency – a bill under the parent/legal guardians name with the physical address (No P.O. Boxes) or State ID/Driver’s license with current address, notarized letter from homeowner that the person (s) resides within their home, paycheck stub with physical address.
Legal guardian must provide guardianship documentation.
Child’s immunization record or a copy.
$10.00 fee per visit/per child CASH ONLY (Services will not be denied for inability to pay with proof of residency)
Bring proof of insurance, if available.
Patients without the above information will not be seen – No exceptions
